The short answer: Print the list below blank and fill it in by pen, or type into it on screen and print it. It has room for allergies, the doctor, the pharmacy, an emergency contact and ten medicines, and you can add rows.

Medication listBring this list to every appointment and to the pharmacy.
Medicine and strengthWhat it is forHow much and whenPrescribed byStartedNotes

Include prescriptions, vitamins, supplements, eye drops, inhalers, creams and anything bought without a prescription. Cross off a medicine when it stops and write the date.

How to fill it in

  • Use one sheet per person, and write their name and date of birth at the top.
  • Copy each medicine's name and strength exactly as the pharmacy label shows it.
  • Write "what it is for" in the person's own words, such as "blood pressure" or "sleep".
  • Copy "how much and when" from the label, including any instructions about food.
  • List vitamins, supplements and over-the-counter products too. Doctors and pharmacists need to know about them.
  • Change the "updated on" date every time you edit the list.

Where to keep copies

Keep one copy at home where a visiting family member or paramedics can find it, such as on the fridge. Keep a second copy in the bag that goes to appointments. Give one to anyone who helps with medicines, and replace all of them when something changes.

Ask the pharmacist to check the finished list against their records. For more on building and keeping the list, read how to keep track of a parent's medications.

This list is kept in this browser so you can come back to it later. On a shared or public computer, press "Clear the list" when you are done.
